Continuous / Mobile Treatment Teams
Continuous Treatment provides assertive outreach to persons suffering from mental illness who are experiencing a level of distress that requires ongoing supervision, but poses no significant danger that warrants inpatient hospitalization. The multi-disciplinary team designs and implements a treatment plan that provides comprehensive treatment in the client’s home.
Goals for Clients:
- To engage in available services
- Remain in the community and establish better lives for themselves
- Develop independent living skills
- Gain emotional stability
- Remain in stable housing
Eligibility Criteria:
- Need for ongoing supervision in order to remain in one’s community setting
- Need for assistance integrating into the community
- DSM-IV diagnosis
- Continued supervision is needed for activities of daily living/social skills, medication compliance, aftercare follow-up supervision
- Client has a safe, stable, therapeutic living environment. The team may need to assist client in finding appropriate housing.
