Fellowship Health Resources, Inc. Chester County (FHRCC) is a service based upon the concept of a Single Clinical Home (SCH). Conveniently located on West Bridge Street, in Phoenixville, the program offers clients a broad range of comprehensive services, both on-site and in the clients home environment that were formerly not available within a single program. Key components of the model include: Outpatient Services, Targeted Case Management, Resource Coordination, Intensive Case Management, a Psychiatric Rehabilitation Day Program, and a CTT (Community Treatment Team).

Treatment:
To ensure that services are individualized and specific to the clients unique needs, a comprehensive diagnostic evaluation is conducted upon admission. The evaluation is multi-faceted and includes a thorough examination of the clients health, psychological, and social situation.
The Single Clinical Home team employs a strengths-based approach to services that is solution-focused and fosters empowerment. Consumers learn to adapt to and manage their illness, regain control of their lives, and improve the overall quality of their personal and social relationships. An alliance, consisting of the client, any family that the client wishes, and the Single Clinical Home team, is established to augment this process.
